Ohio Pesticide Application Recordkeeping Requirements

Commercial Pesticide Applications (Excluding Category 7 & 10)

  • Develop records for all pesticide applications in English
  • Records contain required information:
    • Name of responsible licensed commercial applicator and names of trained serviceperson applying pesticides
    • Name and address of person contracting for service
    • Date of application
    • Type and size of area to be treated
    • Location or field identification number of treatment area
    • Trade name (brand name) and EPA registration number of pesticides used
    • Total amount of each pesticide product used
    • If diluted, total volume of use dilution applied
    • Type of equipment used
    • Time of day of application, start and completion (or when ceased for the day)
    • Wind direction and velocity, air temperature and other weather conditions when applicable
  • Submit records to pesticide business registered location or employer within 10 days of application
  • Pesticide business registered location or employer retains records for 3 years from the date of application and make them available to ODA.

Commercial Pesticide Application (Category 7 & 10, only)

  • Develop records for all pesticide applications in English.
  • Records contain required information:
    • Name of licensed responsible commercial applicator and names of trained servicepersons applying pesticides
    • Name and address of person contracting for service
    • Date of application
    • Pests to be controlled
    • Locations and methods of treatment for each pesticide used
    • Trade name (Brand name) and EPA registration number of pesticides used
    • Total amount of each pesticide product used
    • Application concentration or dilution used for each pesticide and total volume applied
    • Time of day of application, start and completion (or when ceased for the day)

Wood-Destroying Insect (WDI) Inspections

  • Record the results of WDI insepections for real estate transactions on the NPMA-33 form
  • The NPMA-33 form must be completed and signed within 5 days of the inspection
  • The WDI inspection is to be conducted and reported in accordance with rules and regulations
  • The WDI report is to be submitted to the pesticide business registered location within 10 days of completion
  • The WDI report is retained for 3 years at the business location and made available when requested by the ODA
