Form (above) is for:

• Commercial applicator - ($35) License application for applicators employed with a private company, pesticide business, government/state agency, school district, etc. License process
• Business license - ($35) For business that applies pesticides to the property of another for hire, or solicits and takes orders to apply pesticides or conducts diagnostic inspections for wood-destroying insects in a contemplated real estate transaction.
• Pesticide registered location - ($35) License for a branch location of a business that already has a pesticide business license.
• Pesticide dealer - ($35) Sells restricted-use products to licensed commercial or private applicators.
